Qelizat Langerhans jan\u00eb zbuluar p\u00ebr her\u00eb t\u00eb par\u00eb n\u00eb vitin 1868 nga nj\u00eb student gjerman i mjek\u00ebsis\u00eb, Paul Langerhans, nga i cili edhe e kan\u00eb marr emrin. Vler\u00ebsimi \u00ebsht\u00eb q\u00eb shfaqet n\u00eb 1 gjer 2 raste n\u00eb 100.000 persona.<br \/>\nEtiologjia:<br \/>\nMutacionet somatike t\u00eb gjenit BRAF jan\u00eb identifikuar te rreth gjysma e pacient\u00ebve. Mutacionet somatike fitohen gjat\u00eb jet\u00ebs dhe jan\u00eb prezente vet\u00ebm te disa qeliza. K\u00ebta ndryshime nuk trash\u00ebgohen.<br \/>\nGjeni BRAF e kontrollon prodhimin e proteinave, t\u00eb cilat marrin pjes\u00eb n\u00eb koordinimin gjat\u00eb shum\u00ebzimit dhe rritjes s\u00eb qelizave. Mutacionet somatike shkaktojn\u00eb q\u00eb n\u00eb qelizat me mutacion proteina BRAF t\u00eb jet\u00eb aktiv t\u00ebr\u00eb koh\u00ebn madje edhe kur nuk ka sinjale kimike q\u00eb e aktivojn\u00eb at\u00eb. Proteina e tejaktivuar mund t\u00eb ndihmoj\u00eb n\u00eb zhvillim t\u00eb Langerhans histiocitoz\u00ebs, duke u mund\u00ebsuar qelizave Langerhans t\u00eb shum\u00ebzohen dhe t\u00eb rrriten n\u00eb m\u00ebnyr\u00eb t\u00eb pakontrolluar. Gjithashtu jan\u00eb identifikuar edhe ndryshime n\u00eb gjenet tjera t\u00eb qelizave Langerhans. Besohet q\u00eb faktor\u00eb tjer\u00eb si\u00e7 jan\u00eb infeksionet virale dhe toksinat mund t\u00eb luajn\u00eb rol n\u00eb zhvillimin e k\u00ebsaj s\u00ebmundjeje komplekse. Histiocitoza Langerhans zakonisht nuk \u00ebsht\u00eb trash\u00ebguese dhe paraqitet te njer\u00ebz t\u00eb cil\u00ebt nuk e kan\u00eb s\u00ebmundjen n\u00eb familje. Por gjer tani jan\u00eb identifikuar disa familje me k\u00ebt\u00eb s\u00ebmundje, por m\u00ebnyra e trash\u00ebgimit nuk \u00ebsht\u00eb e njohur.<br \/>\nPasqyra klinike:<br \/>\nManifestimi i simptomave dhe shenjave klinike \u00ebsht\u00eb shum\u00eb polimorf tek personat me Histiocitoz\u00ebn Langerhans. Granulomet e eshtrave formohen te rreth 80% e pacient\u00ebve, duke shkaktuar dhimbje dhe fryrje. Granulomet q\u00eb zakonisht shfaqen ne kafk\u00eb ose n\u00eb eshtrat e gjata t\u00eb gjymtyr\u00ebve mund t\u00eb shkaktojn\u00eb fraktura t\u00eb eshtrave. Granulomet q\u00eb paraqiten n\u00eb l\u00ebkur\u00eb si flluska, ose dermatit mund t\u00eb jen\u00eb m\u00eb pak ose m\u00eb shum\u00eb t\u00eb shprehura. Gjithashtu mund t\u00eb kete ndikim edhe n\u00eb hipofiz\u00eb (gj\u00ebnd\u00ebr q\u00eb gjendet n\u00eb baz\u00ebn e trurit e cila sekreton hormone q\u00eb kontrollojn\u00eb shum\u00eb funksione t\u00eb r\u00ebnd\u00ebsishme). Tek rreth dy t\u00eb tretat e t\u00eb rriturve s\u00ebmundja shkakton d\u00ebmtime vet\u00ebm n\u00eb mushk\u00ebri.<br \/>\nMetodat diagnostike:<br \/>\nDaignoza p\u00ebrcaktohet n\u00eb baz\u00eb t\u00eb analiz\u00ebs histologjike dhe imunohistokimike t\u00eb indeve t\u00eb p\u00ebrfshira (qelizat Langerhans jan\u00eb pozitive p\u00ebr neuroproteinat C-100 dhe antigjenin CD1 kurs\u00eb n\u00eb mikroskop elektronik shihen granulat Birbek).<br \/>\nTe adult\u00ebt ku s\u00ebmundja p\u00ebrfshin vet\u00ebm mushk\u00ebrit\u00eb, skaneri i mushk\u00ebrive tregon ndryshime tipike radiologjike q\u00eb mund\u00ebsojn\u00eb p\u00ebrcaktimin e diagnoz\u00ebs.<br \/>\nTrajtimi:<br \/>\nP\u00ebrzgjedhja e qasjes terapeutike varet nga stadi i s\u00ebmundjes, i cili v\u00ebrtetohet p\u00ebrmes ekzaminimeve (statusi fizikal, hemograma, testet e funksionit t\u00eb m\u00eblqis\u00eb dhe radiografija e eshtrave dhe mushk\u00ebrive).<br \/>\nP\u00ebr trajtim p\u00ebrdoren protokole nd\u00ebrkomb\u00ebtare t\u00eb unifikuara duke p\u00ebrfshir\u00eb polikimioterapi, kortikosteroide dhe interferon.<br \/>\nN\u00ebse \u00ebsht\u00eb i p\u00ebrfshir\u00eb vet\u00ebm nj\u00eb organ aplikohet trajtim lokal.
