Incidenca e SATM (mielitit akut sekondar transversal) \u00ebsht\u00eb i ndryshuesh\u00ebm dhe varet nga s\u00ebmundja themelore.<\/p>\n<p><strong>P\u00ebrshkrimi klinik:<\/strong><\/p>\n<p>Mosha e fillimit dhe ecuria klinike variojn\u00eb n\u00eb var\u00ebsi t\u00eb form\u00ebs. Manifestimet klinike t\u00eb p\u00ebrbashk\u00ebta p\u00ebr t\u00eb dyja format p\u00ebrfshijn\u00eb p\u00ebrfshirjen motorike (dob\u00ebsi e ekstremiteteve, ngurt\u00ebsim dhe spazma muskulore me funksion t\u00eb d\u00ebmtuar t\u00eb frym\u00ebmarrjes n\u00eb rastet me p\u00ebrfshirje t\u00eb palc\u00ebs s\u00eb sip\u00ebrme t\u00eb shtyll\u00ebs kurrizore), manifestime senzore (dhimbje shpine, parestezi, mpirje dhe dhimbje neuropatike) dhe gjetje autonome ( mosfunksionim seksual, nxitje\/mbajtje urinare, nxitje\/mbajtje e zorr\u00ebve dhe disrefleksia autonome). Shenjat e tjera varen nga s\u00ebmundjet shoq\u00ebruese n\u00eb SATM (mieliti akut sekondar transversal) q\u00eb p\u00ebrfshin nervat kraniale dhe periferike. IATM (mieliti akut transversal idiopatik) ka nj\u00eb ecuri monofazike nd\u00ebrsa SATM (mieliti akut sekondar transversal) mund t\u00eb p\u00ebrs\u00ebritet.<\/p>\n<p><strong>Etiologjia:<\/strong><\/p>\n<p>IATM (mieliti akut transversal idiopatik) besohet t\u00eb jet\u00eb nj\u00eb s\u00ebmundje e nd\u00ebrmjet\u00ebsuar nga imuniteti p\u00ebr shkak t\u00eb nj\u00eb reagimi imunitar t\u00eb vont\u00eb kund\u00ebr nj\u00eb infeksioni t\u00eb koh\u00ebve t\u00eb fundit q\u00eb synon pa dashje palc\u00ebn kurrizore. SATM (mieliti transversal akut sekondar ) \u00ebsht\u00eb p\u00ebr shkak t\u00eb nj\u00eb s\u00ebmundjeje inflamatore: skleroz\u00ebs s\u00eb shum\u00ebfisht\u00eb (MS), neuromielitit optik (NMO), lupus eritematoz sistemik (SLE) dhe sindrom\u00ebs Sjogren, dhe gjithashtu mund t\u00eb jet\u00eb p\u00ebr shkak t\u00eb nj\u00eb s\u00ebmundjeje infektive (bakteriale, parazitare ose virale).<\/p>\n<p><strong>Metodat diagnostikuese:<\/strong><\/p>\n<p>N\u00eb diagnoz\u00eb, kryhet nj\u00eb MRI e palc\u00ebs kurrizore p\u00ebr t\u00eb konfirmuar pranin\u00eb e mielitit akut t\u00ebrthor dhe shkakun, n\u00ebse ka nj\u00eb t\u00eb till\u00eb. Testet laboratorike jan\u00eb gjithashtu thelb\u00ebsore p\u00ebr t\u00eb identifikuar shkakun e SATM (Mieliti akut transversal sekondar): Analiza e CSF mund t\u00eb zbuloj\u00eb pranin\u00eb e brezave oligoklonal (kryesisht t\u00eb pranish\u00ebm tek MS) ose infeksioneve bakteriale\/virale\/parazitare; serologjia mund t\u00eb zbuloj\u00eb pranin\u00eb e autoantitrupave ndaj aquaporin-4 (NMO), antitrupave anti-ADN (SLE) ose antitrupave anti-Ro\/SS-A (sindroma Sjogren).<\/p>\n<p><strong>Diagnoza diferenciale:<\/strong><\/p>\n<p>Diagnoza diferenciale p\u00ebrfshin mielopati akute transversale t\u00eb shoq\u00ebruar me fistula, trauma dhe lezione akute kompresive si metastazat dhe abscesi epidural.<\/p>\n<p><strong>Menaxhimi dhe trajtimi:<\/strong><\/p>\n<p>Qasjet e trajtimit varen nga prania e s\u00ebmundjes shoq\u00ebruese. Trajtimi akut mund t\u00eb p\u00ebrfshij\u00eb kortikosteroidet dhe shk\u00ebmbimin e plazm\u00ebs n\u00eb pacient\u00ebt rezistent ndaj steroideve. P\u00ebrfitimi i imunoglobulinave intravenoze dhe ciklofosfamidit \u00ebsht\u00eb ende duke u p\u00ebrcaktuar. Kujdesi afatgjat\u00eb p\u00ebrfshin menaxhimin simptomatik, rehabilitimin dhe, n\u00eb rastin e SATM, trajtimin afatgjat\u00eb t\u00eb mir\u00ebmbajtjes p\u00ebr t\u00eb parandaluar rikthimet ose p\u00ebrparimin e s\u00ebmundjes.<\/p>\n<p><strong>Prognoza:<\/strong><\/p>\n<p>Prognoza \u00ebsht\u00eb e ndryshueshme, e paparashikueshme dhe varet nga p\u00ebrgjigja ndaj trajtimit.
